Nonsurgical Management of Unruptured Tubal Pregnancy with Transvaginal Aspiration and Methotrexatc Injection






Abstract
One case of unruptured tubal pregnancy-was managed conservatively with the transvaginal aspiration and injection of methotrexate(MTX) into the intact tubal gestational sac with the use of transvaginal ultrasonography under local anesthesia. The patient was followed with the serial measurement of serum ?-hCG levels and transvaginal ultrasonography.
The patient had experienced no serious complication or side effects of MTX. Resorption and disorganization of the gestational sac became increasingly obvious within 3 days of the procedure without evidence of bleeding. After 24 days, serum -hCG level decreased to 63 mIU/mI and ultrasonography showed a mixed echogenic mass measuring 2X 3 cm which was invisible 9 months later. Spontaneous menstruation began within 6 weeks of the procedure.
These observations suggest that the nonsurgical conservative management with the transvaginal aspiration and injection of MTX may become a new, effective and safe treatment modality for the unruptured tubal pregnancy.
